Did you make sure to use redsn0w to "Deactivate" your device after installing SAM?

I heard about pushfix and push doctore but have not tried any of them, SAM i have tried on many devices (3GS and 3G) with no problems, perhaps i can help you get SAM running if you describe SAM behavious on your device and steps you took to enable its activation.

I jailbroke 4 devices on 4.2.1 and I only got this issue in one of them so I am going to try and restore and rejailbreak again cause it seems there is something wrong and no one can clarify... Good luck
